2. Read and understand the Code of Ethics and Professional Conduct. You will be asked to attest to your understanding and agreement with this in the application.
3. To apply for your membership the required forms are listed below.

• Consultant Member: $553.70.
  (Includes $440 membership fee, a $50 non refundable application fee and 13% HST)
• Supplier Liason Group Member: $802.30.
  (Includes $660 membership fee, a $50 non refundable application fee and 13% HST)
• Affiliate Consultant Member: $305.10.
  (Includes $220 membership fee, a $50 non refundable application fee and 13% HST)

Your application will be promptly acknowledged and presented to the Board of Directors which is responsible for final approval of all membership applications.
